Diagnostic Tools

Finally put words to what you’ve been feeling

“This is what’s been going on for me.”

This collection of diagnostic-style tools includes:

  • ADHD Questionnaire

  • Generalized Anxiety (GAD) Questionnaire

  • Major Depressive Disorder (MDD) Questionnaire

  • Borderline Personality Disorder (BPD) Questionnaire

Each questionnaire was thoughtfully created using DSM criteria, current research, insights from leading experts, and my clinical experience as a therapist.

These are not surface-level checklists.
They’re designed to help you see yourself more clearly your patterns, your struggles, and even your strengths.

While these tools are not standardized assessments and do not provide a formal diagnosis, they are powerful in helping you:

  • Understand what you’re experiencing

  • Feel validated in your symptoms

  • Communicate more effectively with your therapist, doctor, psychiatrist, or provider

Think of this as your starting point
a way to walk into your next session with more clarity.

And when you understand yourself better, you can start getting the kind of support that actually helps.
